We understand that as exotic pets are becoming much less “exotic”, many practices are receiving a growing number of inquiries for exotic specialists who can handle more advanced cases and surgeries. As one of the Midwest’s most accomplished practitioners in the care of exotic animals, Dr. Teresa Bradley Bays enjoys helping exotic patients in any sort of consulting or referral capacity.

Referral Information

If you are a veterinarian and would like to refer a case to Dr. Bradley Bays, please rest assured that we will keep you apprised of all stages of the treatment process. We provide follow-up documentation for all cases and you are welcome to contact us at any time with questions.

The below forms will give you an idea of our referral process. Please fill out the Patient Referral Form so that we can understand the patient’s history and health concerns. We will send you a completed version of the Patient Referral Update after we examine or treat your pet.

Patient Referral Form – If you are a referred patient or a referring veterinarian, please fill out following form before the patient’s first appointment.

Patient Referral Update – An example form of the patient referral update a primary veterinarian will receive post-treatment or consultation. Dr. Bradley Bays will fill out all relevant fields in as much detail as possible.
